Information Sheets & Forms
There will be occasions when our administrator, Alberta Pensions Services Corporation (APS), will need information from you to keep your pension record current or to make a pension transaction on your behalf. For example, APS may need to verify your date of birth, you may want to transfer your pension benefits from another plan, provided a reciprocal transfer agreement exists between the plans, or retire.
Instructions
- Print the form and complete it by hand.
- Sign and date the form.
- To be valid, the paper copy of the form must be signed by hand, in ink. No form of electronic signature is acceptable for pension purposes.
- Send the form to PSPP's administrator, Alberta Pensions Services Corporation (APS):
