There will be occasions when our administrator, Alberta Pensions Services Corporation (APS), will need information from you to keep your pension record current or to make a pension transaction on your behalf. For example, APS may need to verify your date of birth, you may want to transfer your pension benefits from another plan, provided a reciprocal transfer agreement exists between the plans, or retire.

Instructions

  1. Print the form and complete it by hand.
  2. Sign and date the form.
  3. To be valid, the paper copy of the form must be signed by hand, in ink. No form of electronic signature is acceptable for pension purposes.
  4. Send the form to PSPP's administrator, Alberta Pensions Services Corporation (APS):
