Information Sheets & Forms

There will be occasions when our administrator, Alberta Pensions Services Corporation (APS), will need information from you to keep your pension record current or to make a pension transaction on your behalf. For example, APS may need to verify your date of birth if you want to transfer your pension benefits from another plan, provided a reciprocal transfer agreement exists between the plans, or retire.

The forms needed for these situations are listed below, among others. For your convenience, we have included information sheets with most of the forms. Read the information carefully, then:

  • Print the form and complete it by hand.
  • Sign and date the form.
  • Send the form to APS at the address provided at the bottom of the form.

To be valid, the paper copy of the form must be signed by hand, in ink. No form of electronic signature is acceptable for pension purposes.

You will need Adobe Acrobat Reader to view the files if you do not already have it. The reader is available free of charge from the Adobe website at www.adobe.com.
